CCT Course for Family Caregivers at Sutter Gould Health Education Center

LOCATION: Sutter Gould Health Education Center
 at 1700 McHenry Ave., Suite 60B, Modesto, CA 
in the McHenry Village Shopping Center

DATES and TIMES: Eight Thursdays, April 30 – June 18, 2015 from 6:00 pm – 8:00 pm (last class will be from 6:00 – 9:00pm)
If you are admitted to the course, you must show up to the first meeting or your space will be offered to someone on the wait list.

FEE: No fee – fee is paid for by the Stanislaus County Area Agency on Aging. You must be a caregiver of an elderly family member in order to attend this course.

COURSE DESCRIPTION: Please read the information about this course. In particular, please be aware of participation requirements. If you anticipate missing a class, please wait to take this course at a time when you can attend all of the classes. You will given a pre-course assignment two weeks before the class meets for the first time, please allow for this preliminary preparation for the course. Some supplemental materials will be made available on this website – it would be helpful to have the use of a computer or smartphone to access these supplemental materials – if this is a hardship, these materials can be provided.

APPLICATION: Applications are now being taken. The application period will end at COB (close of business) on Mar. 27, 2015. Please note that the class size is limited to 20 people. A lottery will be held to give everyone equal opportunity to have a chance to take this course.  Everyone who applied by the deadline will be notified by April 1st. If you are not selected, you will be placed on a wait list. Should someone cancel before the first class, those on the wait list will be notified of an opening, in the order they are on the wait list. You should receive a reply that your application has been received within 2 business days. If you have not received a reply, please send an inquiry.

It is recognized that the trainings may at times make participants feel vulnerable. CCT is not a therapeutic intervention for psycho-pathological disorders as defined in the DSM, and is not meant to be used as such. By completing this release and consent I assume all risk for any physical or mental consequences of participating in the course. By signing this release I also specifically and expressly agree to hold harmless, indemnify and release Lori Wong, Stanford University, CCARE, CCT, and all training staff and each of their respective employees, partners, agents and associates from any and all liability for the results of the educational guidance that will be or has been provided. Moreover, I acknowledge that the particulars shared in this class by other participants are to be maintained as confidential. I will not blog or otherwise report on content shared in this class in any form of media. I also acknowledge that taking this course does not entitle participants to teach course content or represent themselves as affiliates to CCARE/Stanford University.
